Grade 9 Immunizations are scheduled for Monday, February 2 at SRT. Your son/daughter will be bringing home a consent form either today or tomorrow. Consent forms must be signed and returned to the office by Monday, January 26. Please note that consent forms can also be done online, please see the link in the attached document.
A second immunization date is scheduled for Monday, February 9 for those that miss the initial date. Any questions regarding immunization should be directed to the health unit by calling 604.476.7000.
Students in grades 10-12 who have missed previous immunizations will be able to get those done on Monday, February 9. If you are not sure which immunization(s) are outstanding, you can call the Maple Ridge Health Unit at 604.476.7000
